As the EMA Licensing Branch has informed you, all persons applying for an EMA Licence in BC must undergo a criminal record check with a vulnerable sector screening, paid for by the applicant, each time they apply for a new licence or a licence reinstatement.

The Branch needs your consent to confirm the accuracy and completeness of the information you declare. Personal information on this form is collected by the EMA Licensing Board under the authority of the Freedom of Information and Protection of Privacy Act Section 26(c), the Emergency Health Services Act (section 6), and the Emergency Medical Assistant Regulation. This information will be used to issue you an EMA licence and maintain a permanent register of licensed EMA’s. This information is protected from unauthorized use and disclosure in accordance with the Freedom of Information and Protection of Privacy Act and may be disclosed only in accordance with that Act.

By clicking the “Start My Check” button below, I acknowledge, agree, and consent to and with the EMA Licensing Board of BC, in writing, to all of the following:

  1. The Board collecting from sources other than me, information about any charges or convictions that I may have, including by access to the criminal record history that I declare to Sterling Talent Solutions;
  2. The Board using and disclosing information about my charges or convictions for the purpose of processing the licence application, for the purpose of administering any licence that is issued, and for all purposes of the Board’s activities, including those relating to licensing, quality assurance, practice review, complaints, investigations and disciplinary action;
  3. The Board, if it considers it necessary to do so, disclosing to the licence applicant information that reveals the existence, and particulars, of my charges or convictions, for the purpose of enabling the licence applicant to revise or re-submit the licence application with or without my involvement in the application;
  4. The Board storing or accessing my charge or conviction information inside Canada for the purposes described above.

This consent is effective starting on this date and does not expire.
